The film is a series of the dramatic series TBS 2021 “Tokyo Mer: Mobile Room” and the film 2023 “Tokyo Mer: Mobile Urgery Room the Movie”. In the new version of the film, a Nankai mobile emergency room is established. They travel by ferry to distant islands and have a special vehicle equipped with a operating room. A volcano then broke out on a neighboring island. Meanwhile, more members of the distribution for the film were announced and they are Ayami Nakajo, Shinya Kote, Hayato Sano, Jesse Lewis and Phongchi.

Draw the synopsis by Asianwiki: In 2025, a trial of Nankai Mer began. The Nankai Mer (mobile emergency room) moves around the islands in the South Sea on a ferry and transports the special NK1 vehicle which is equipped with an operating room. Doctor Kota Kitami (Ryohei Suzuki) and nurse Natsume Kuramae (Nanao), who are both from Tokyo Sea, are assigned to Nankai Mer. One day, a large eruption suddenly occurs on the island of Suwanosejima in Kagoshima. The hot lava descends to the villages and flying volcanic rocks destroy everything on its way. Due to the smoke from the volcano, helicopter rescue is impossible. In this desperate situation, Nankai Mer is challenged to save 79 people blocked on the island.
